Our mission is to simplify access to this essential medication through comprehensive support and guidance.

This support program is available in all provinces in Canada, except Quebec. Only patients using SIVEM Imatinib are eligible.

How to enroll a patient?

To enroll a patient, the pharmacist should download and complete the form below. Once completed, the form can be faxed to 1-855-817-4836 or emailed to sivemhorizon@patientassistance.ca. Our dedicated specialist will then reach out to the patient within one business day to finalize their enrollment.
